I sent mine in from the actual magazine, and it was a fairly small form, so the sizing on that form from the link looks about right. HTH
The fine print on this offer says "actual certificate and copy of receipt must accompany request" and "offer many not be reprinted without written permission"...I'm assuming this means an original form is needed. Was this in the January All You? I have that around here somewhere...

I ordered mine a few weeks ago, came very quickly and had a lot of $1 off Q's that I used in this sale. Definitely worth getting I thought!
Is it $25 before or after coupons/sale price? I did the Con deal last week but did not spend $25 out of pocket after coupons would that receipt work?
What kind of expiration date do these coupons have?

I also sent for mine awhile ago and got it in the mail yesterday. I used some of the coupons today. All of the coupons have a June 30, 2010 expiration date.
Does this offer only apply for the brands that are shown on this rebate form? Are there more Conagra brands that would also apply for this rebate? Just wondering if we can include more products than we know of now.
